Dear all: Here is the revised pilot questionnaire based on the feedback I've gotten. 
  • It's designed to send out in advance, then to be also used as a template for the personal interviews. 
  • I've added at the end questions suggested about lessons learned, communication impediments, etc.  
  • I made changes to refer to city/county to address the concerns about Montgomery County.  It's a bit awkward stylistically, but I thought it better than having two separate questionnaire versions made up, one for counties and another for cities. 
  • I added a question about unique jurisdictional issues (such as multi-state, federal district, etc.); this will be an issue beyond Montgomery county, I suspect. 
  • Likewise, I put in the sniper incident as an example of emergency incident but also left the weather example.  Since we're developing this for a broad range of communities, I feel the need for a range of examples that make sense across the country. 
  • It's now 7 pages long, which is pushing the limit for busy people, as noted. 
Of course, since this is a pilot, we'll have a chance to get feedback on whether we need tighter/broader coverage, shorter/longer series of questions.
